Hi, my name is Oksana Knapp and I work for Sowing Seeds of Health. My title is Lead Care Manager and Community Liaison. Sowing Seeds of Health is an Enhanced Care Management Provider for members with Partnership. Enhanced Care Management (ECM) is a new statewide Medi-Cal benefit. The goal of ECM is to provide a whole-person approach to care that addresses the clinical and social needs of vulnerable populations. Each member who enrolls will have access to a single Lead Care Manager who provides Comprehensive Care Management and coordinates their health and health-related care and services.

Members can get connections to the quality care they need, no matter where members seek care—at the doctor, the dentist, with a social worker, or at a community center. LCM’s can connect members to Community Supports which can offer support to secure and maintain housing, get access to medically tailored meals to support short term recovery and a variety of other community-based services. Each Lead Care Manager is a certified Community Health Worker. A CHW is a frontline public health worker who is a trusted member of the community, often from the same community they serve. They work to improve health outcomes and reduce health disparities by connecting people to resources, promoting health education, and advocating for their needs. CHW’s play a crucial role in bridging the gap between healthcare systems and communities, particularly those with limited access to services.

We serve every age, from children to elders. Anyone interested in ECM can self refer themselves. We aim to be a leading ECM Provider in all the counties we serve. We currently serve SHASTA | BUTTE | YOLO | GLENN | COLUSA | SONOMA | NAPA | MARIN | HUMBOLDT | PLACER | SUTTER | MENDOCINO | YUBA| Counties. We are currently hiring for Lead Care Managers to join us in Marin and Sonoma County!
